Add user specific information to Control Center web login and dashboard pages Merged

Control Center web console login page is currently a base page of nothing but a login and password with bare minimum information. The ability is needed to be able to customize that page for our environment.

Warning banner for the user that this is a monitored system and unauthorized use is not permitted is needed.

Configurable instance name, so the user knows which Control Center the login page is for. The only way to currently determine this is by the URL presented. There are multiple production environments, let alone the non-production instances that could be presented.

Once connected, the Dashboard page gives no indication as to which Control Center instance you are logged into. Again, only the URL can be used to determine which instance is currently being displayed.
